9月に入って涼しくなってきましたね。
9月といえば、食欲の秋・読書の秋・スポーツの秋です!
スタバの新作が出てました。その名も ‘おさつバターフラペチーノ’ !!
まだ飲めてないので、早く飲まないと!!
秋の食べ物も美味しくてついつい食べすぎるので、運動を始めようと思っています。
今週は看護部からの投稿です。
今回は問診についてお話しします。
初診で来院された方は、まず問診票を記入していただきます。
その後、看護師で再度対面での問診をして、ドクターの診察という流れになっています。
看護師の問診は、問診票を元に症状の確認を行います。記入内容と重複してお話をさせていただくこともありますが、お話の中で思い出される方もいらっしゃるため、つらい症状をよく理解するためにもご了承ください。また、他院受診歴や、検査歴のある方は受診・検査歴をメモなどにまとめていただくとスムーズに問診が可能となるのでよろしければ持参をお願いいたします。

This week, let’s discuss the topic of medical history taking.
For first-time visitors, the process typically begins with filling out a medical history questionnaire. Following that, there is a face-to-face interview with a nurse, followed by a doctor’s examination.
The nurse’s interview is designed to confirm and elaborate on the information provided in the questionnaire. Although some questions might overlap with what’s on the form, this process is essential for a comprehensive understanding of your symptoms. Sometimes, during the conversation, patients may recall additional details about their conditions, which can be crucial for accurate diagnosis and treatment planning.
For those who have visited other healthcare facilities or undergone medical tests in the past, it’s helpful to prepare a summary of your medical history, including previous treatments and test results. Bringing this information, perhaps in the form of notes, can greatly expedite the interview process.

当院の問診票の特徴としては、スポーツや趣味の欄があります。
なぜ、趣味までと疑問を持たれる方もいると思いますが現在の辛い症状に関係していたり、治療の目標となることもあります。以前されていた事でも、始めたばかりの事でも構いませんので記入をして頂ければと思います。お仕事の内容も差し支えない範囲での記入をお願いします。
現在多くの外国人の方が来日されています。
当院でも開院後定期通院をされている外国籍の患者さんがおられます。
もし怪我などをした場合、病院受診が不安と思われる方もいるかと思います。
英語の問診票も用意しています。
(院長はカタコトですが、その他複数のスタッフが英語での対応が可能です)
One distinctive feature of our medical history questionnaire at our clinic is the inclusion of a section for sports and hobbies. You might wonder why we ask about hobbies in relation to your current symptoms, but it can be relevant to understanding your condition or even becoming a part of your treatment goals. Whether it’s something you’ve been doing for a while or just recently started, we encourage you to fill it out. Please feel free to provide information about your hobbies within the bounds of your comfort. Please also provide information about your job within the limits of what you’re comfortable sharing.
Currently, there are many foreigners residing in Japan, and some of them are our regular patients who have been visiting our clinic after opening. We understand that if you ever have an injury or any medical concerns, seeking medical attention at a hospital might be daunting, especially if you’re more comfortable communicating in English. Therefore, we have also prepared an English medical history questionnaire. While our director may have limited English proficiency, we have some staff members assisting you in English.
